**REMEMBER - you are not put on the official MOM calendar until we receive your $100 deposit and your registration form. 

If you are a Team Leader and have already contacted the office about a mission trip, you may download the official team forms here.  Team leaders only should be downloading these forms so that each member has the proper form on file.

Please send the ORIGINALS into the office and keep a copy for yourself.
